Had a blue screen of death, image goes black every once in a while, numerous problems

I had my first blue screen of death a few hours ago while playing counter-strike go. I have no idea what caused it and I'm worried about my pc. Every now and then my display will go black, I'm not sure why that is. Even during games it will happen. That happens after my pc has been on for a while, at least 8 hours. I haven't cleaned it in months, and I've had it for nearly 2 and a half years.

Haven't installed anything at all today. Haven't installed any hardware in the past two years.

My specs:

Windows 7 64 bit home premium
AMD Phenom II Quad-core processor clocked at 3.82ghz
HIS Radeon 6950
Corsair PSU 750w
Gigabyte motherboard
16gb ram


What the blue screen looked like:

My graphics overdrive settings on the AMD catalyst were:
750MHz with fan control at 69%.

I now have 500MHz with fan control at 100% because I'm thinking it was a heat issue. The highest temperature I saw for my graphics card went up to 70 degrees while I was running CSGO in windowed mode. My PC crashed and restarted almost a week ago while I was running CSGO, and 3 weeks ago my pc did it around 3 or 4 times while I was running GTA IV.
