Start Menu Icon Error, Save as Type Text Missing

Hello, everyone. First post and I have a problem.

I have a Windows 7 Toshiba Laptop, Service Pack 1.

A few months ago, I tried to change all the folder icons on my laptop to this cool black design. I used the registry to do this, and it worked-sort of. Unfortunately, it only affects Icons used in certain view types, which I do not use. So I decided rather than deal with inconsistency, I would just restore the default icons. This worked, but now I have two separate problems and I believe that both are connected to this.

First, sometimes Icons are missing. Right now, for example, my Malwarebytes Icon on the Start Menu is displaying that White Paper with a little Window on it, rather than the appropriate icon. This also happens to any folder on my Start menu (under All Programs) that I highlight. Sometimes it also happens when I am viewing the preview of a window from the Task Bar. How do I fix this?

My second problem is that when I try to save files (only seems to affect Pictures) there is no text in the Save as Type field. It is empty, and if I click on it then all I see is All Types under the blank field. What causes this and what do I do about it?
